Summary

A Windows CLI installer can leave descendants running after its primary process exits. This adds an internal Windows x64 helper that owns the process tree through a Job object and reports a terminal result only after the original primary handle is signaled and the Job's active-process count reaches zero.

Creation is suspended until Job assignment succeeds. STARTUPINFOEXW restricts inherited handles to owned standard handles, and a private overlapped named pipe drains bounded output without blocking synchronous pipe polling. Wait/query failures remain unknown; exit code 259 is a valid exit code.

Installer and pack policies add no deadline, process-count cap or kill-on-close behavior. The version-probe policy alone uses the existing eight-second deadline and separate one-second reap window. Failure paths retain owned lifetime until it can be reconciled and close each acquired handle once.

Published against #7596 at d10c5c9d38e5026b9f7f4825687e6e3644929994, targeting codex/windows-cli-provenance-stack-20261023. The eleven-file helper/fixture delta and validation below describe that recorded parent. The parent has since advanced; its current integration and the eventual retarget to dev remain separate work for the existing stack owner.

Scope and follow-up

No production consumer imports this helper yet, and Windows apply remains disabled. Integration must adopt ownership before every pack/install/probe spawn and retain the lease and staging while lifetime is unknown. This supplies one prerequisite for #2811; it is not complete Windows update support.

This is process-lifetime ownership, not an OS sandbox. ARM64 is unsupported. WMI-created escape processes and reverse inheritance by an unrelated unrestricted CreateProcess call are outside the guarantee.

Verification

  • Current candidate ac3522266a3c258341c9ca499bfe53e7ce4a692d normally merges published head 0a4eb35a85007f7ad3e6c5948fe5150df92024e3 with feat(codex): add proof-bound Windows CLI update planning #7596 at d10c5c9d38e5026b9f7f4825687e6e3644929994. The five shared documentation/registration files preserve both the parent's Worker entries and this PR's entries. The native child implementation, ABI, fixtures and tests are byte-identical to the published child.
  • Runtime and configured test runner are now both Bun 1.4.4.
  • Historical source-unit evidence at 0a4eb35a85007f7ad3e6c5948fe5150df92024e3, official Bun 1.4.4, native Windows x64, bun test --isolate --timeout 60000: tests/codex-integration/codex-cli-windows-child.test.ts 17 passed / 0 failed / 300 assertions, including the actual owned native child Job/pipe and reap-deadline cases; the cached-PAC reconnect case in tests/clients/desktop-compatibility-runtime.test.ts passed. Those unchanged runtime cases were not rerun.
  • On this exact merged tree, official Bun 1.4.4 full-project TypeScript, structure SSOT, skill-surface and eleven-file privacy/size checks passed, as did git diff --check. Focused layout placement/membership checks passed 6 tests / 12 assertions (23 unrelated tooling cases filtered out). All five validation processes exited 0 with complete output.
  • The new duration-table row is a scheduling sample from a successful Bun 1.4.4 integration run, not a measurement of every current parent head.
  • Runtime/test-runner, root/GUI dependency inputs, preload and the relevant fixture/helper inputs match the published child. No installation was performed; existing runtime passes are reused as source-unit evidence, not fresh merged-head execution.
  • Historical fan-in head (feat(codex): apply Windows CLI updates with owned transactions #7648, bc08a484), which contains this change, passed 510 tests / 64 platform skips / 0 failures / 2,911 assertions across 18 units on the earlier validation. It predates this Worker-parent refresh and does not qualify the new combined head.
  • Current-head automatic CI 40141602834 completed successfully on ac3522266a3c258341c9ca499bfe53e7ce4a692d (PR attempt 1): 12 successful jobs / 11 skipped / 0 failures, including all four Ubuntu shards, gates, structure and aggregate CI. Windows, macOS and Desktop jobs were skipped. This is not an exact-head lane=all result.
  • This helper still has no production consumer in this PR. Refreshed parent/fan-in integration and explicit maintainer security acceptance remain outstanding. POSIX and ARM64 support are outside this Windows x64 helper's scope. No live app, service, account or installation was changed.

Review readiness and merge conditions

Marked Ready for review on 2026-10-28 at unchanged head ac3522266a3c258341c9ca499bfe53e7ce4a692d, after rechecking its successful automatic CI and absence of submitted inline review findings. This opens the existing implementation for review; it is not merge approval, a parent refresh, or new native acceptance. The existing reviewer requests are retained. The earlier Draft-related automated-review skip is not counted as a completed review.

  • The updated updater-stack decision superseded the earlier blanket post-release deferral with conditional asynchronous progress. Review can start while independent security review, required fixes, parent/dev integration, duplicate-sibling review and the applicable exact-head full-platform validation remain separate landing conditions.
  • This child's verified parent remains the recorded d10c5c9d. The parent branch has advanced since that validation; the PR metadata's historical base SHA and clean flag do not prove integration with its current tip. The previous dda17d86 comparison was a historical snapshot, not the current parent claim.
  • The previously inspected feat(codex): apply Windows CLI updates with owned transactions #7648 fan-in already included this head and later transaction integration. Its prior 1b608880 snapshot is not asserted to be current. The existing stack owner retains source refresh, final retargeting and duplicate-landing decisions; no fan-in files were copied into this standalone helper.

No source changes, unchanged test reruns, manual workflow dispatches, branch rewrites, merges, deployments or operational setting changes were performed by this readiness update.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.
  • Current parent/fan-in integration and applicable full-platform validation accepted by the stack owner.
  • Explicit maintainer security acceptance before merge.

Carry only the request-id, POSIX job-store and status usage hunks from 86ad5a0; Windows read-only plan tests remain separate.
…lans

Port the Windows-plan hunks of 44353b9, 86ad5a0 and c291ff8 onto the
7586 POSIX stack head 4c22308. A proof-captured absolute standard npm
codex.cmd shim, its global-prefix package, prefix-local Node/npm and the
prefix-local optional backing are bound by held-handle identity
re-verification; the plan then reports applicable: false with refusal
windows_apply_disabled. PATH-only selection and separate-prefix Node stay
refused; Windows apply remains disabled. The independent
cli-installation-targets refused-probe fix (#7577) is intentionally not
included.
Opt-in helper for the disabled Windows CLI apply path (#2811). Starts an
explicit drive-absolute .exe via CreateProcessW(CREATE_SUSPENDED), assigns it
to an unnamed non-inheritable Job, then resumes. STARTUPINFOEXW whitelists
exactly the three owned inheritable stdio handles; output flows over a private
single-principal overlapped named pipe. Terminal proof requires the original
primary handle signaled plus a successful Job accounting query reporting zero
active processes; WAIT_FAILED, query failure, PID absence, acknowledged
termination and exit 259 are handled exactly. Installer and pack policies add
no deadline, process cap, abort kill or kill-on-close; only the version probe
keeps its existing 16 KiB / 8 s / 1 s reap policy. No consumer imports it yet;
Windows apply remains refused and ARM64 is unsupported.

Walkthrough

The pull request adds validated POSIX Codex CLI updates with durable jobs, worker execution, artifact verification, and management routes. It adds restricted Windows x64 inspection and read-only planning. It also updates Bun 1.4.4 documentation and test coverage.

Changes: Codex CLI update flow

Layer / File(s) Summary
Planning and artifact verification
src/codex/cli-update-plan.ts, src/codex/cli-update-artifacts.ts, src/codex/cli-update-files.ts
Planning becomes asynchronous and binds verified package artifacts, archive contents, installation preimages, and plan identity.
POSIX application and durable jobs
src/codex/cli-update-apply.ts, src/codex/cli-update-job.ts, src/codex/cli-update-lease.ts
Apply validates the plan, artifacts, installation, process state, lease, and readback. Jobs support durable admission, retries, deduplication, lifecycle status, and bounded storage.
Workers and selection coordination
src/codex/cli-update-job-worker.ts, src/codex/cli-update-worker.ts, src/codex/cli-update-selection-gate.ts
Workers support correlated replies, cancellation, commit fencing, close settlement, and diagnostics. Shared gates coordinate updates with selection mutations, restarts, runtime writes, shims, and workspace startup.
CLI and management integration
src/cli/codex-cli-update.ts, src/server/management/codex-cli-update-routes.ts, src/server/management-api.ts, src/server/management/route-registry.ts
The CLI and management API expose plan, apply, and status. HTTP apply requires trusted loopback GUI-session access and explicit confirmation.
Windows inspection and process support
src/codex/cli-windows-plan-provenance.ts, src/codex/windows-cli-child.ts, src/codex/windows-cli-child-pipe.ts, src/codex/windows-cli-child-native.ts
Supported Windows x64 npm layouts can produce managed read-only plans with applicable: false and windows_apply_disabled. Native child and pipe modules provide bounded process observation.
Documentation, capability declarations, and tests
docs-site/src/content/docs/reference/*, structure/codex-cli-update.md, src/cli/capabilities-base.ts, tests/codex-integration/*
Documentation and capability declarations describe the new flows. Tests cover planning, application, jobs, workers, Windows behavior, routes, and Bun runner selection.

Before merging, fix the Windows output-pipe handling and stop refused apply attempts from permanently filling the update job ledger. The Windows helper is not yet used in production, but the POSIX apply path could become unusable after repeated refused attempts. Runtime resolution can also throw while an update is committing. The other findings are small cleanups and documentation corrections.

OpenCodex #7586: the management plan/apply paths ran synchronous
full-preimage hashing, file-set walks and process scans on the HTTP
event loop. Each admitted job now runs in a fresh Worker realm
(cli-update-worker in WORKER_ENTRIES) while admission and the
generation-fenced durable ledger stay in the parent.

- per-job SAB fence PREPARING->CANCELLED/COMMITTED plus a real
  AbortSignal driven by a cancel message; the commit CAS is the last
  guard before the installer spawn, and a post-COMMITTED cancel defers
- one process-wide shared selection gate covers the short commit
  window: the five mutable selection writers, the remote-workspace
  spawn and the desktop restart ladder report busy instead of racing
- the run message carries the allowlisted frozen launcher snapshot;
  the worker rebuilds inspection deps and derives fresh selection
  evidence in its own realm
- settlement requires a valid reply AND the worker close; the host
  never terminates a worker, so post-delivery loss settles ambiguous
  with the lease retained and live workers tracked

Base: b0e2d8d
Guard worker entry installation with Bun.isMainThread, remove message handlers for natural drain, and preserve explicit spawnWorker coverage. Add fresh-main import and real Bun child-drain regressions without forced worker termination.

Release decision for v2.93.0: DEFERRED_POST_RELEASE. This PR is part of the Codex CLI updater stack (#7586 → #7596 → #7626 → #7648, siblings #7614/#7618/#7619), which is deferred as a whole until after v2.93.0. The evidence and the suggested landing order are in #7586 (comment). #7648 already contains this branch as an ancestor.

Update the check capability detail: Windows check now performs candidate file I/O.

The second details entry on Line 581 still says: "On Windows this first slice performs no candidate or configuration filesystem I/O: only a proof-captured absolute environment candidate can receive lexical app-bundle or version-manager labels; every other Windows candidate fails closed."

This PR changes that behavior:

  • src/codex/cli-install-provenance.ts Lines 1111-1116 send every non-excluded absolute Windows candidate to inspectWindowsCodexCliPlanInstall.
  • src/codex/cli-windows-plan-provenance.ts then opens node.exe, the manifests, codex.cmd and the backing package through inspectWindowsInstallationFiles (Lines 69, 97-100, 117, 132-144).
  • A standard npm codex.cmd can now be reported as managed: true.

Automation reads ocx capabilities as the declared surface index. The detail now gives agents a wrong no-I/O guarantee. The generated skills/ocx/references/01_surface_observe-system.md Line 352 carries the same text. Regenerate it with bun run skill:surface after this fix.

Update the stale rationale in the Windows exclusion comment.

Lines 1090-1091 say that "Windows inspection currently refuses before candidate I/O". That statement is no longer true in this PR. In src/codex/cli-install-provenance.ts Lines 1111-1116, the Windows branch now calls inspectWindowsCodexCliPlanInstall. That function opens handle-bound files through inspectWindowsInstallationFiles (src/codex/cli-windows-plan-provenance.ts Lines 69, 97-100, 132-144).

The code is still correct. The real reason for the exclusion is the 32,767-character Windows environment-block budget: start keeps the inherited environment, so the snapshot would duplicate PATH in it. A stale comment can lead a future maintainer to enable the snapshot for Windows start on a wrong premise. It can also hide the actual effect: a Windows ocx start server never has a snapshot, so the management plan route always reports windows_inspection_deferred there.

Remove the contradiction with the attest paragraph on Line 305.

Line 290 now says that Windows x64 check inspects a standard npm codex.cmd through held handles. The unchanged last sentence on Line 305 still says: "Le check Windows existant ne réalise toujours aucune E/S de fichiers candidats ou de configuration." The two statements now contradict each other on the same page.

The English source (docs-site/src/content/docs/reference/cli/agents.md Line 1246) replaced that sentence with "The separate Windows check/plan predicate above can establish read-only planning evidence; attest remains observation-only." The path instructions require that translated locale pages not contradict the English source.

The French page also has no section for the new apply/status commands (English Lines 1203-1233). A pointer to the English section would keep readers from concluding that the update surface is read-only.

📝 Proposed fix for Line 305

As per path instructions: "Check that user-facing docs stay in sync with actual CLI/API behavior and that translated locale pages (ja, ko, ru, zh-cn) are not left contradicting the English source."

A failed lease setup leaves a permanently busy installation-prefix lock that this call created

acquireCodexCliApplyLease creates <prefix>/.ocx-codex-cli-apply.lock at Line 45. It then opens owner.json at Line 51 and writes it at Line 53. Two failure cases leave the directory behind:

  • openSync throws at Line 51 (for example ENOSPC, EMFILE or EACCES). ownerIdentity stays undefined, so Line 65 skips release(). The new empty directory stays in place.
  • writeFileSync throws after a partial write at Line 53. release() then fails its identity.size !== Buffer.byteLength(record) / content check at Lines 26-28, so the partial owner file and the directory both stay in place.

In both cases every later applyCodexCliUpdate for that prefix returns installation_busy. The cause is a one-time transient I/O error in this call, not another owner, and no code path recovers it. The operator must remove the directory by hand. The comment at Lines 63-64 says this is intentional for "empty or suspicious directories". That reasoning covers directories that another actor left. It does not cover a directory whose dev/ino this call recorded in lockIdentity a moment earlier.

The job store already handles the same situation correctly. rollbackInitializingLock in src/codex/cli-update-job.ts (Lines 134-156) removes only the directory and partial owner file whose identities the failing call proved. The tests at tests/cli/cli-codex-cli-update.test.ts Lines 197-260 cover that behavior. Apply the same identity-fenced rollback here. The fix also needs a regression test that injects an owner openSync failure and then expects a second acquireCodexCliApplyLease(prefix) to return acquired.

clearPersistedCodexRuntime now throws, and its unguarded caller in runtime resolution passes that throw to every resolver caller

Before this change, clearPersistedCodexRuntime never threw. The try/catch around unlinkSync handled every error, and the deleted-lines evidence shows that the contract was "warn once, never fail". This change adds a throw at Line 427 that sits outside that catch: throw new CodexCliSelectionMutationBusyError() runs whenever the shared gate is held.

The internal caller at Line 1434 does not guard the new throw:

else if (result.runtime.source === "fallback" && persistedRuntime?.command) {
  ...
  if (pinVanished) clearPersistedCodexRuntime(deps);   // Line 1434, no try/catch
}

Compare the persistCodexRuntime branch three lines above it (Lines 1410-1417). That branch catches the same busy error and turns it into persistError. The retire-vanished-pin branch does not. Trigger:

  1. A Codex App update replaces the hashed plugin directory that the persisted pin names. This is the #4035 scenario that this branch exists for.
  2. In the same process, a cli-update worker holds WORKER_COMMIT. This lasts from commitWindow.tryEnter()/waitEnter() through the final awaited scanCodexSessionProcessesAsync and the installer spawn. Any other parent writer that holds PARENT_WRITE has the same effect, for example a desktop restart that runs for up to 20 s per shell.
  3. Any runtime resolution in that window throws codex_cli_selection_busy out of persistResolvedCodexRuntime, so the error reaches resolveCodexRuntime* callers. Examples are CodexRemoteWorkspaceRuntimeFactory.available()/start() (src/remote-control/workspace-codex-runtime.ts Lines 307, 318) and the status/doctor paths. Previously those callers got a fallback result.

The retirement step is opportunistic. If it is skipped once, the next resolve retires the pin. Fix it at the caller and keep the explicit-call contract (busy is an error) for direct users:

🐛 Proposed fix in persistResolvedCodexRuntime (Line 1434)
-    if (pinVanished) clearPersistedCodexRuntime(deps);
+    if (pinVanished) {
+      try { clearPersistedCodexRuntime(deps); }
+      catch (error) {
+        // A held cli-update commit window defers retirement to the next resolve;
+ // it must never turn a fallback resolution into a thrown error.
+        if (!(error instanceof CodexCliSelectionMutationBusyError)) throw error;
+      }
+    }

Add a regression test for this case. Hold createWorkerCommitWindow(...).tryEnter(), resolve with a vanished pin, and expect a fallback result rather than a throw.

Do not treat a successful zero-byte read as end of stream.

Line 66 sets this.eof = true whenever a read completes with done === 0. The Windows ReadFile contract says a TRUE completion with zero bytes on a pipe means the peer called WriteFile with zero bytes. End of stream is ERROR_BROKEN_PIPE (109), and finish() already handles that case without setting error.

Once eof is set, pump() returns at line 42, so no further reads are issued. Output the child writes afterward stays in the pipe buffer and is not captured. When that buffer fills, the child's WriteFile blocks.

For installer and pack, deadlineMs is null and terminate is false (src/codex/windows-cli-child.ts:9-10). The stop paths at windows-cli-child.ts:94, :98, and :140 therefore do not run, so the parent does not stop the child.

   private finish(error: number): void { this.eof = true; this.error = error !== 109; }

The loop stays bounded. pump() runs at most four iterations, and a zero-byte completion now falls through to the next read.

Add a native fixture case. The child calls WriteFile with zero bytes, writes more than 64 KiB, and then exits. Assert that wait() returns terminal: true with the full captured output under the pack policy.

GET /api/codex/cli-update/plan starts a new Worker and registry/process work for every request, with no single-flight

Each request to this route calls runCodexCliUpdatePlanInWorker(). The call creates a new Worker realm (src/codex/cli-update-job-worker.ts Lines 298-302). That Worker then runs createCodexCliUpdatePlan, which does the following:

  • A bounded content hash of the installation, including a native backing of up to 1 GiB (MAX_NATIVE_BACKING_BYTES).
  • Three isolated npm view --json child processes.
  • A full process-table scan.

The route has no in-flight deduplication and no concurrency cap. Repeated dashboard refreshes, several open dashboard tabs, or an admin-token script looping on this GET each start a separate Worker and set of npm children. The host never calls terminate() on a Worker, so any stalled npm query keeps its Worker and children alive until the per-query deadline expires.

The plan is read-only, and the route takes no client input that changes it (channel is always "latest"). Concurrent callers can therefore share one in-flight promise without changing the result:

♻️ Single-flight the plan dispatch
 const STATUS = /^\/api\/codex\/cli-update\/jobs\/([0-9a-f-]{36})$/;
+let inflightPlan: Promise<Awaited<ReturnType<typeof runCodexCliUpdatePlanInWorker>>> | null = null;

 export async function handleCodexCliUpdateRoutes(ctx: ManagementContext): Promise<Response | null> {
   if (ctx.url.pathname === "/api/codex/cli-update/plan") {
     if (ctx.req.method !== "GET") return jsonResponse({ error: "method_not_allowed" }, 405, ctx.req, ctx.config);
     try {
-      const plan = await runCodexCliUpdatePlanInWorker();
+      inflightPlan ??= runCodexCliUpdatePlanInWorker().finally(() => { inflightPlan = null; });
+      const plan = await inflightPlan;
       return jsonResponse(plan, 200, ctx.req, ctx.config);
     } catch { return jsonResponse({ error: "plan_unavailable" }, 503, ctx.req, ctx.config); }
   }

The test "plan dispatch failure returns HTTP 503 without a second dispatch" in tests/server/management-route-registry.test.ts still holds with this change. finally resets the slot, so a later request retries.

Map the gated parent modules to codex-cli-update.md, which now defines their gate contract

The section "Worker realm and the selection commit window" in structure/codex-cli-update.md (Lines 242-261) now owns a contract that seven parent operations must follow. It names their files:

  • src/codex/runtime.ts
  • src/codex/shim-overlay.ts
  • src/remote-control/workspace-codex-runtime.ts
  • src/codex/desktop-app-restart.ts

The doc also cites src/codex/cli-windows-plan-provenance.ts, src/codex/windows-installation-files.ts and src/codex/cli-installation-limits.ts. This documents array omits all of these files. The generated structure/INDEX.md therefore maps src/codex/runtime.ts only to catalog-effort-clamp.md (INDEX Line 229).

The repository rule says "Changing an area obliges the same change to review every doc listed for it". With the current map, a later edit that removes or reorders beginCodexCliSelectionParentWrite in persistCodexRuntime, the shim boundaries, the remote-workspace spawn or the desktop restart does not send the author to the only doc that states the "exactly seven parent operations" invariant. The gate accepts these additions, because the doc already names each path.
